March 24, 2014: Morris Webb, 42, of Brockton, Massachusetts, has died after sustaining serious personal injuries in an auto accident in Brockton at approximately 1:15 a.m. on Saturday.  Joshua Cooper, 25, also of Brockton, was operating a vehicle on Main Street when he allegedly ran a red light and crashed into the side of Webb’s car, which was traveling on Legion Parkway.

The impact caused Webb’s vehicle to roll over and ejected him in the process.  He was transported to Signature Healthcare Brockton Hospital, where he later succumbed to his injuries.  Cooper and his passenger were also injured and taken to Brockton Hospital.

No criminal charges have been filed yet and the accident is currently under investigation by the Massachusetts State Police Accident Reconstruction Team and the Plymouth County District Attorney’s Office.

March 14, 2014: Dennis Canning, 64, of Middleborough, Massachusetts, and Kimberly Bebare, 30, of Ames, Iowa, died on Tuesday, as a result of serous personal injuries sustained in a head on collision.  The crash reportedly took place between 1:00 and 1:30 p.m. on Route 44 in Middleborough, near a Holiday Inn Express hotel.

Canning and Bebare were operating separate vehicles in opposite directions, prior to the collision.  Both drivers were pronounced dead at the scene of the accident.  While neither driver had any passengers, a dog in Bebare’s car survived and remains in the care of Middleborough’s animal control officer.

Police are currently investigating the cause of the accident.

December 10, 2013 – Charles Hoffman, 71, of Easthampton, Massachusetts has died as the result of serious personal injuries sustained when his pickup was involved in a head car crash with a van driven by Mauricio Velasquez, 32, of Springfield. Velasquez’ three year old daughter, who was a passenger in the van him, also died. The accident occurred on Hendrick Street in Easthampton at approximately 3:30 p.m. on Saturday.

Hoffman and the three year old girl were transported by ambulance to Baystate Medical Center, where they were pronounced dead upon their arrival to the hospital. Hoffman’s wife, Janice Hoffman, 69, Velasquez, and another passenger in his car, Milder Velasquez, 25, were also taken to Baystate and treated for their injuries. Janice Hoffman remains in critical condition.

The Northwestern’s District Attorney’s Office stated Hoffman’s pickup was struck when Velazquez lost control of his van, hit the guard rail and crossed over the center line into oncoming traffic. Velasquez has been charged with several offenses, including motor vehicle homicide while under the influence of alcohol in violation of G.L. c. 90 § 24G, two counts of operating a vehicle under the influence of alcohol resulting in serious injury in violation of G.L. c. 90 § 24L, and several civil traffic violations.

Mr. Velasquez was arraigned on Monday in Springfield District Court and his bail was set at $100,000.

November 19, 2013 – Lynne Cabral, 27, of Dartmouth, Massachusetts, and Marilha Lopes, 26, of Fall River, sustained serious personal injuries as the result of a five car crash. The accident occurred on Route 24 North near exit 13B in Raynham on Monday morning.

Cabral and Lopes were operating separate vehicles at the time of the accident. Both were transported to Good Samaritan Medical Center in Brockton, where they were treated for non-life threatening injuries.

The Massachusetts State Police have stated that the other three drivers, a 62 year old male from Swansea, a 20 year old Fall River woman, and a 33 year old male from Stoughton, were not injured.

According to police, the crash occurred when the five vehicles rear ended one another. The cause of the accident is unknown and the investigation is ongoing.

November 6, 2013 – Elaina Millard, 42, of Tewksbury, Massachusetts, has died as a result of serious personal injuries sustained when a car struck her as she attempted to cross the street. The accident took place at the intersection of Main Street and Summer Street in Tewksbury at approximately 6 a.m. on Tuesday.

According to the Tewksbury Police Department, Millard was crossing the northbound lane of Main Street at the time of the accident. She was transported by helicopter to Saints Memorial Hospital in Lowell, where she was later pronounced dead. The male driver, who has yet to be identified, was not injured.

The crash is currently under investigation.

October 23, 2013 – Scott Cintron, 51, of Taunton, Massachusetts, has died as the result of serious personal injuries sustained when his car crashed into a utility pole. Robert Scibilio, 24, also from Taunton, was a passenger in Cintron’s car and was seriously injured in the accident. The crash occurred on Hart Street in Taunton at approximately 3:30 p.m. on Tuesday.

According to the Taunton Police Department, Cintron was pronounced dead at the scene. Scibilio was transported by medical helicopter to Massachusetts General Hospital, where he continues to receive treatment for his injuries.

Witnesses to the accident stated that Cintron was driving down a hill and traveled over the sidewalk prior to smashing into the utility pole. Cintron’s vehicle split in half as a result of the collision.

The accident remains under investigation.

October 21, 2013 – Richard Johnson, of Hopedale, Massachusetts was airlifted to UMass Memorial Medical Center after he sustained serious personal injuries when his motorcycle collided with a pickup truck. The accident occurred on Wednesday at approximately 7:15 pm on Route 16 in Mendon.

The Mendon Police Department has stated that Johnson suffered significant injuries, but is expected to survive. The truck driver’s identity has not been released and no charges have been filed.

The accident remains under investigation.

September 25, 2013 – Susan Macchi, 64, and Juliet Macchi, 23, both of Carver, Massachusetts, have died as a result of serious personal injuries sustained in a head on collision. The accident occurred at approximately 12:29 a.m. Sunday morning on Federal Furnace Road in Plymouth.

Susan Macchi was driving with her daughter Juliet, when their vehicle collided with another car driven by a 25 year old Kingston man, whom the Plymouth Police Department have yet to identify. The man was found lying on the side of the road when police arrived on scene, but only sustained hip and pelvic injuries. Susan Macchi was pronounced dead at the scene. Juliet Macchi was transported to Jordan Hospital by ambulance, where she later died.

No charges have been filed, although police have stated that alcohol was not a factor in the accident. The investigation is ongoing.

August 26, 2013 – Marjorie Carter, 88, of Carver, Massachusetts has died as a result of serious personal injuries sustained when her car was involved in an accident with a truck driven by Carl Jewells, 46, of North Attleborough. The crash occurred at about 4 pm Sunday on Route 58 in Carver.

Stephanie Waite, 19, was riding as a passenger in Jewells truck. All three were injured and taken to Jordan Hospital. Carter died while on the way to the hospital.

The Carver Police Department stated that no charges have been filed, but the accident remains under investigation.

August 19, 2013 — Miles Tibbetts, 16, of Wellfleet, Massachusetts, has died as the result of serious personal injuries sustained when his bicycle was struck by a vehicle driven by Dorothy Herold, 76, also of Wellfleet. At the time of the accident, Tibbetts was attempting to cross over Route 6 in Wellfleet. The crash occurred at approximately 12:30 p.m. on Saturday.

Tibbetts was unresponsive at the scene. He was transported to Cape Cod Hospital in Barnstable and was later flown by medical helicopter to Beth Israel Deaconess Medical Center in Boston, where he was pronounced dead later that day.

Tibbetts was going to be a junior at Nauset Regional High School where he was a member of the wrestling team. His friends and family mourn his loss.

The accident remains under investigation by the Wellfleet Police Department and they have not said whether criminal charges will be filed against Herold.
